**A Family Membership provides coverage for you, your spouse, and dependents under the age of 26 who reside in your household. Does not include multiple families living at the same address.
The 2025 Ambulance Membership Plan provides coverage for March 1, 2026, through February 28, 2027. Forms must be submitted by July 1, 2026.
Coverage is for HOPS Ambulance Association only and does not cover transports by other EMS organizations.
Please note: Membership applies to HOPS services only. Any services provided by paramedics (Guthrie EMS, Greater Valley EMS, Western EMS) or other practitioners, such as other local ambulance agencies, may be billed separately. Your membership means that you will not be held responsible for emergency transport above and beyond your insurance reimbursement unless we are required by law to bill you. You will be held responsible for any insurance payments for ambulance services not forwarded to the agency.
Applications for membership can be found below. It is important for all families and individuals to have a membership in a local ambulance association, as insurance coverage does not always cover the entire cost of an ambulance call, leaving a balance due after insurance coverage. We reserve the right for third-party billing.
